That only occurs for the FA Cup. If it's say Man City as in your example, the place goes to next highest club in Prem league,

wrong, the runner up in BOTH cups qualify if the winner has gained a european spot by some other means,
(top 4, FA cup, FA cup runner up, fair play winner)

Good point that. The Carling Cup is a Football League Competition so why are they bending over backwards to reward Premiership teams who don't even take the Competition seriously?
I have Emailed the FL and suggested that they should change their rules so that the highest placed FL team left in the Competition at the S/F stage should get the Europa League Place, no matter who wins it. I have also suggested that the change should happen now to avoid a terrible injustice
