Whoever was in charge of sorting the Insurance for Sala should not be in their position now if they failed to do it.

This worries me a little.

It was my understanding that the club confirmed, within their accounts, that the money was set aside. If the confirmation was made, but the money not set aside, might the club be leaving itself open to allegations of false accounting?

This is not my area. Maybe Keith could be asked to comment.

Surely not

I understand that the club set aside a provision in the accounts, which means its already accounted for. However, that doesn't mean that the money was set aside, just that it's already been priced as a loss.
